Please bring the following to your first rheumatology visit:
1.Please bring your insurance card and photo ID. If your insurance company requires a referral for
your first visit, please make sure your referring doctor has sent it to us. We’ll verify your
insurance coverage and benefits and let you know any out-of-pocket expenses we will collect at
the time of the visit and file insurance claims on your behalf.
2.If we can’t validate your referral or insurance coverage prior to your first appointment, we may
need to reschedule your appointment.
3.Any previous lab and/or radiographic X-ray/ultrasound/MRI tests results for review (medical
records are typically sent by the referring physician, but occasionally – despite best intentions –
are not present. Sometimes tests need to be repeated to confirm the result)
4.An up-to-date medication list with the specific dosages you are taking (include a list of
medications you have already tried to reduce duplication of prior treatments)
5.A list of allergies to medications
Your family history, including any known relatives with rheumatologic/autoimmune disease At your first appointment, you’ll be asked to complete several patient forms. You can save time
by printing and completing them in advance and bringing them with you.
