Our staff will call you before your appointment. If you are unable to make it, please let us know so we can reschedule.
- On your appointment day, arrive ten minutes early.
- Bring a picture ID and your insurance card.
- Bring all recent lab reports and XRAY reports.
- Bring a list of all medications, prescribed and over-the-counter, and supplements.
